Insurance

Please send an email if you’re a prospective client and we will forward you a copy of the initial paperwork. The info is useful for treatment planning, your informed consent, billing insurance, and clarifying office policies.

We work with the following in-network insurance plans:

Cigna

The Health Plan
Tri-Care
United Healthcare
UPMC

 Always check with your insurer, as some programs have multiple plans that reimburse out of network providers (often at a different rate),  and some have multiple plans that may or may not contract with us.  Finally, ask about Copay and Deductible information.  Some insurance programs have a lofty deductible that will require you pay the first 100-1500 dollars before they pick up the charges.

Cancellation Policy: A 24 hour notice is needed if you are unable to keep your appointment.  Clients can be billed for all appointments missed without 24 hour notice.
